Ryan Brothers Ambulance, established in 1962, a family owned ambulance service, has expanded its model to support Accountable Care and Triple Aim goals with the addition of educated Community Paramedics. As primary care extenders deployed to a patient’s home, CP’s carry out the orders of a primary care plan and assist with chronic disease management and prevention. Post- discharge follow up by a CP within 48 hours has proven to reduce readmissions, thereby improving the health of an identified population and reducing the costs and penalties to hospitals.  Ryan Brothers Ambulance believes this is the EMS model of the future.
